The PCC Network provides a collaborative framework that aims to facilitate integration of the polyclinics and private clinics, thereby providing a holistic care to patients at the primary care level.
Collaboration is a key component of any integrated framework. To facilitate collaboration, SHP has made available several resources on this website to share with the doctors in the private sector:
Listing of GP clinics.
The clinics are currently listed according to their neighbourhoods. The network will feature search engines in the next phase.
Compilation of resources on flu pandemic.
The resources include updated reading materials on flu pandemic, MOH standard operating procedures and guidelines etc
Avian flu clinic notices
The polyclinics and GP clinic would need to put up key messages to patients during times of national healthcare emergencies like a flu pandemic. SHP can coordinate with MOH in coming up with key messages. SHP will then design and put up various collateral and make them available on the PCC network for GPs access and usage.
SHP Medication Identification Aid
Medication ID Aid (MedIA) is a project by SHP that provides details of over 110 drugs. The details include digital pictures of the drug, its external packaging, physical description (shape, colour, inscription, etc), manufacturer, brand and generic names and MOH drug classification. Doctors are able to search for drugs by drug class or by colour of medication.
This project was developed to assist doctors to determine the medications that patients, especially the elderly and illiterate patients, are taking. Doctors can access MedIA and show patients the aforementioned pictorial information on drugs via their computers and get patient’s verification of medication on the spot. Doing so cuts down consultation time spent on eliciting drug information and it also enhances patient safety.
Doctor’s Clinical Guidebook
SHP Doctor’s Clinical Guidebook on selected conditions are featured on the PCC network for the GPs’ easy reference on symptoms of conditions, SOPs in treating certain conditions and others.

Abstracts of SHP scientific publications and research projects are posted on the website. This enables SHP to share its research findings and results of improvement projects with the GPs.
