The Partners in Community Care Network, or PCC Network, was established in 2008 by SingHealth Polyclinics (SHP) to function as a coordinating body with the clusters of general practitioner clinics and dental practitioner clinics located near the nine SingHealth polyclinics. 

SHP aims to engage the private community doctors, to explore ways to collaborate as well as to share resources such as clinical materials and best practices etc, to provide holistic care to patients in the community.
